Anticipated Start and End Dates: June 2023 – end of December 2023
Job Address: 3297 Washington St, Jamaica Plain, MA 02130
Hours Per Week: 4-7 hours/week (150 total hours)
Work Schedule: varies/flexible
Pay Rate and Travel Stipend: $18/hour and one-time $240 travel stipend
Internship Class: If chosen for the internship, you will take the three credit INT-299 Learn and Earn internship course during the summer semester (you pay for the course but if an internship course is not part of your program, we can try to see if a required course can be replaced but this is not always possible). 
 
 
Organization Overview:
Brookside Community Health Center on Washington Street in Jamaica Plain was founded in the 1970s and now serves almost 12,000 patients from all over Boston. Our patients mostly reside in Jamaica Plain, Roxbury, Dorchester, Roslindale, Mattapan and Hyde Park but many come from other neighborhoods in Boston and surrounding communities. We have adult medical, pediatric, dental, obstetric/gynecology, nutrition and mental health departments. Many of our patients have been coming to see Brookside providers since they were born and we have many families where 4 generations are all taken care of at Brookside. 73% of our patients identify as Latinx, 15% as Caucasian and 10% as African American. Spanish is the preferred language for at least 60% of our patients. Our staff reflects a similar mix of Boston neighborhoods, cultures and languages. In addition to clinical staff like doctors, nurses, therapists and medical assistants, many other staff work in administrative capacities to support our patients, such as office management, financial and managed care assistance, social work, and community resources.
 
Job Description: 
 
The health coach will establish collaborative relationships with Latin-x patients with pre-diabetes in order to identify and support patients’ health goals. The health coach will learn motivational interviewing and confidence-promoting techniques to help patients better manage health goals. They will work with a clinical team including physicians, nurses and other health providers such as nutritionists and resource specialists, for example. The health coach will work with 10-15 patients for 6 months. They are expected to contact patients at least once a week by phone, text or video chat to assess progress on patients’ goals. The coach will learn to document their meetings in the electronic medical record. The health coach will meet with program leadership at least every 2 weeks to discuss developments and challenges.

If selected for this internship, the intern will need to complete the following:
·       Show a copy of government issued photo ID
·       Completed Immunization records including TB results (received within the last 90 days) *Complete immunization records and proof of two Tb tests, one within a year and one no older than 90 days (2 TB tests are REQUIRED).
